I had a similar run as well. My 95 was not shifting from 3 to 4, changed the fluid and filter and still nothing. I drove it like that for a few months and then one day “bonk” and no forward gears. Reverse was good long enough to get up a hill to a parking lot then it was the flatbed of shame...

I have a 95 parts truck. If you're not in a real hurry, I can see if can get it out for you. I had bypassed it years ago when the door locks failed, but I think it should still work. Maybe?
If I still had my 95, I'd print that and laminate it. Alas, my 97 is a little different without the distributor, etc. but I'm curious how much so. I haven't owned it long enough to sort through any of that yet. Fortunately.
You may want to look into your distributor etc. I had that problem a while back and it turned out my distributor had gone south. It can be confusing to nail it down, though, so here's the record of my saga on nas-row. Maybe it will help.
Corteco seems to be the most recommended, and Trevor at Rovahfarm has a good deal on them as well as the 2 different o rings you’ll need if you’re re-using your same lines.
Yes, it’s gone to Death Rowver on my buddy’s property where it’s freely offering up its vitals to my replacement ’97. Parts are still available if you need something from a ‘95. I won’t using everything.

It's remarkable how loose/worn the joints can be in the steering column - way before the box becomes an issue. Hopefully, the broken head is a sign of denial by the box to the previoius "fixer."
You may already know this, but these let you use standard shocks on a 2” lift. You can also get matching towers for the front.
