I would try to adjust the kickdown cable first since that is easy. I had similar issues twice and both times they were the cable getting out of adjustment. Don't put the adjustment way out of spec just to get it to shift into 4th though as that may cause problems in the other gears creating wear...
I used Headman Hedders part #39800 on my D1 4.6 swap. For me it was the steering column linkage that got in the way and required some bending of the tube. That and I had to switch to the flat D2 style knock sensor on the driver's side to get the connector proper clearance.
You can try to hit it from below, but you'll most likely just end up blowing PB Blaster in your eyes rather than getting it where it needs to go. Depending on how stuck it is you can use the straw and get under the gaiter on the lever and soak everything, but chances are you'll need to take...
