I think I know why I couldn't shift into 3rd/4th. But what I really want to know is how the hell could this have happened?

Check out how the inside outer ring is all chewed up, all the shiny brass showing. The check out the outer edge and look how it's disformed and "pinched."

There are a few other photos in there as well showing what is going on with the synchro.

Any ideas on what could have caused this to happen?

I'm not seeing any chipped teeth and nothing jiggly inside the main gearbox. But...the short top shaft in the bellhousing (primary pinion????) is a little jiggly. I can grab the shaft and it easily wiggles a tiny bit. According to page 64-C in the green bible the primary pinion bearing can be replaced. Whatcha think?

The primary input shaft bearing will have a small amount of play - remember that the forward end of that input shaft also rides in the bushing in the center of the flywheel so its supported at both ends. I'd look very carefully at the needle bearing between the main shaft and the input shaft. If that's good its probably the brass bushing that 2nd/3rd gears rotate on on the main shaft.
