You can have them made up by a fab shop or simply purchae one that's made for another vehicle, like Cherokee, Wrangler, Landcruiser, etc.. You can easily find one for a Cherokee in the $200 range. Offshore places like Kaymar, TJM are pricy but very good quality. No matter what you use, make sure you have a solid foundation to mount the tire carrier. There's a lot of weight that hinged on one point and that point better be on a solid mount!

Andrew, sorry about the delay in replying about the Kaymar P38A bar - I was away on a trip picking up a second hand Kaymar bar and wheel carrier which, as you surmise, Kaymar no longer make. I've been taking a batch of photos of the bar and will probably trace some of the bits out just in case someone wants to make a replica (I'd never say 'copy'!)
The hardest bits to replicate would be the cast alloy side pieces which fit between the rear wheel and the bar and look like the original pieces in that location. I may make up some fibreglass moulds of these items just in case I ever wreck the castings.
