Thanks for the replies. I have in the past put the truck up on jack stands and run it but i wasn't under it or in the engine compartment.
I've spent more time than I would like to have researching and the three things I'm going to check without the engine running (but still up on stands) are 1-heat shields 2-cat and 3-exhaust manifold to downpipe gasket
I already have an exhaust leak in cylinder 8, and a noisy top end.
I just can't wrap my head around why if the car is rolling, and is in N, and i depress the accelerator I don't get the sound.
And if I do decide to kill myself, i"ll livestream it.
Have the same thing - only in drive and only when cold...after it warms up it goes away. Is yours there all the time? I rev in neutral also and it does not make the noise. I understand the load issue but exhaust is exhaust - air is air under load or not??

If there's a load on the engine, it will torque on its mounts. This can exacerbate an exhaust leak, since the downpipes are held to the body further back. A small or non-existent gap can open up.

You don't hear it in neutral because there is no load on the engine.
