Threw the 2004 SLABS in my 99 D2 this spring. TT's front and rear, CDL. Finally got it into the mountains this last weekend. Lots of loose dirt on rocks. Steep hills.
Night and day difference between the two SLABS. The '04 only kicked in with one wheel in the air and let the TT's do their job the rest of the time.
Previously, the two seemed to be always working against each other.
Well worth it IMO.

Was it plug and play? Or did you have to get it programmed?

DII SLABS are plug and play but if the donor truck had air springs and yours has coils you'll need to get the setting changed (or vice versa). Also if the donor truck had the Three Amigos on and your truck is okay you may need to get the ABS codes cleared, although opinions differ about whether or not that is necessary to keep the Amigos at bay. My read of the RAVE says that if the shuttle valve switches were the fault the codes will need to be cleared but I may be wrong.

Dumb question, what is the SLABS?

Self-Leveling / ABS control unit.

The early ones had issues with being overly sensitive and throwing codes for such things as squeaky brakes, bumps in the road, etc....

LR finally got them sorted, and the latest version works really well, especially if you have CDL.

Looking at purchasing an '00 DII that has been converted with the CDL and the 'O4 SLABS ECU - Is this a pretty common modification to swap the SLABS ECU? Familiar with adding the CDL to the early DII models, but I'm unfamiliar with the SLABS ECU swap.

Maybe not so common, the earlier slabs works fine with cdl, but I believe the later does have an improved control version for TC and no sporadic false faults for bumps in the road and brake noise.
The only downside is that when one of your hubs starts going you won't know it as soon.
I scored a brand new never used slabs off of ebay and I did have to go through an initial set up process for it.

The main problem with the older SLABS and CDL is that if you shut the vehicle off with the centre diff locked the traction control and ABS will be disabled when the vehicle is restarted.

My cdl actuator is electric and powers down with the truck anyway so I didn't have that issue for the most part.
