4th gear ratio incorrect

I know this has been beat pretty good here but still no dice. '04 DII, 85k miles. Locked out of 4th gear.
New MAF, fluid changed, etc. Had hooked up to Autologic scanner on test run & the computer showing in 4 gear the vehicle isn;t engaging & there is no output shaft revo's showing. Otherwise shifting, driving brilliantly.

Any ideas if this is solenoid related or what I could look at next?
