I have seen this mentioned a few times. I was recently searching for a fix for the nipple that comes off the top of the tank and remember seeing discussions where the wrong screw was used to attach the shroud. Maybe try a search as I remember several discussions. If you need a repair for the nipple that goes to the overflow: I have a fix for that.

Yea it is where the bolt is so I'll see what size it is supposed to be and go from there.

more than likely the shroud was replaced and one of the outside (longer) self driving screws was used and it pierced the top of the radiator. You can pull the screw, make a gasket around the hole with rtv, replace screw with it still damp and let it cure.

fyi that is a solution but not a fix. the radiator is likely ruined and will need replacing ultimately.
