Brake pads, front/rear/parking

So looks like I need new brakes. When I was putting the lift on and had my tires off, I noticed the prad pads were awfully thin. This is the first time I've had and used a truck enough to need new pads -- so, how do I change them? never done it before, but I learn quickly.

I assume the front and rear are similiar, but how difficult is it to change/adjust the parking brake? From what I understand, the brake acts on the transmission instead of on the wheels. THe transmission was just replaced in this truck shortly before I bought it. Perhaps its simply not adjusted right, since I can't imagine using it enough to wear it out, but it doesn't hold my truck even the the lever fully applied.

Also, how do tell if your rotors need to be replaced?
