Check me if I'm wrong here but I always start with the simplest solution, did you check for a maimed child?

Once my old lady neighbor came to me and asked if I would check under her car because it made the same terrible noise when it started up. Her fear was that her cat had gotten up there to keep warm and got caught in the fan when she turned her car on. Any cat hair anywhere?
