Tranny or Torque Converter?

Hey guys, I have a 99 D1, 140k mi. on it. A while ago, I was having trouble with the stepper motor, and the engine would start at 2000 rpm and wouldn't kick down for quite a while. As this is my daily driver, it meant there were a few times I would be at a stop light, standing hard on the brake while the motor was running at the 2000 rpm.

Soon after, I had some problems with the tranny shifting back and forth between gears while at a constant speed on level pavement. I changed the fluid and the filter. That erratic shifting is gone, and the tranny has settled into a consistent pattern: Very weak startup from a standing stop (get to about 2000 rpm before the truck starts moving . Once moving it shifts pretty well, but so smoothly that you don't feel it shift at all.

From a cold start (like sitting overnight), it clunks pretty hard when put into reverse. Once it's driven a little, I get the normal clunk shifting into reverse. Shifting into drive, I get no clunk at all. It is engaged, but there is no force to it, if that makes sense.

I am wondering if this is a torque converter issue, or a problem inside the tranny, like clutch plates. I have no experience with trannies, and am trying to figure out what I need to get this repaired.

Any suggestions, ideas etc? I am looking for any guidance (other than driving it to the junkyard!) on this.
