Had a DI that was making a noise like that ended up being one of the front brake pads moving back and forth.

Try some new wires and that will fix it or rule them out!

Sounds like the stepper motor but maybe look for vacuum leaks also that can make the rpm's spike.

At least take the heads to a machine shop and have them check them out for worn valve guides etc. that's a lot of work if there's something that you can't see or even know to look for and you have to do it again 6 months down the road!!!!!
